For months, I only knew that it could either mean "Don't click if you value your sanity" or "There might be a tiny bit of naked human flesh in this image/There might be a swearword somewhere hidden in this text." Those conflicting meanings had me somewhat confused.
I like your icon! Yeah, I've definitely seen that. Maybe there should be a FAQ about proper use of the NSFW tag. My favorite is when people neglect to label / cut stuff that is NSFW. There've been a couple times I was LJing at work, and my internal monologue went something like "Hmm... that's really inter - NIPPLE!" But they haven't fired me yet, so I guess nobody's noticed.
Oh dear. Luckily, my work is largely computerless, or I do it alone at home. But even when I have my laptop with me in the university, I'm always quite careful about placing myself so the screen is covered. The strange stuff you stumble upon even on LJ doesn't need to be seen by everyone.
